Attendees of this one-hour webinar session will gain knowledge on how to better prepare rural health organizations for value-based reimbursement by reviewing the definition and premise behind the reporting of Hierarchical Condition Category (HCC) ICD-10-CM codes to Medicare and other payers, including documentation and coding requirements, and how HCC codes affect beneficiary risk adjustment scores.

We will also review the “ICD-10-CM Official Guidelines for Coding and Reporting” as they relate to the Social Determinants of Health (SDOH) category of “Z-Codes.” These Social Determinants of Health reflect the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. We will discuss methods to support accurate documentation and coding capture for reporting on healthcare claims now and in the future.
